Home: How To Access Services
- The Fife Community Care Team receives a referral from the Children & Families (disability) Team.
- The Team Leader allocates the case to a Social Worker

- The Social Worker arranges to meet Children & Families Social Worker within one month
- The Children & Families (disability) Social Worker meets with the Social Worker Community Care Team to arrange an introduction to the users and/or carers and to agree roles and responsibilities in joint working.

- Full Assessment of need is completed by Social Worker Community Care Team
- Any review meetings held must be attended by either one or both of the allocated workers
- The Social Worker Children & Families (disability) Team introduces Social Worker Community Care Team to User/Carers
- A summary of needs and resource implications discussed with Team Leader and Social Worker(s)
- The Social Worker identifies locally based resource to meet users needs
- The Social Worker requests Post 16 Working Group funding on Proforma (P16P) from the Post 16 Working Group Contract Officer
